Primary noncemented total hip arthroplasty in patients with ankylosing spondylitis. Clinical and radiographic results at an average follow-up period of 6 years.

چکیده
Twenty consecutive primary noncemented total hip arthroplasties performed on 12 men with ankylosing spondylitis were available to be studied clinically and radiographically at an average follow-up period of 75 months (range, 27-121 months). The average patient age at the time of surgery was 35 years (range, 23-53 years). Harris hip scores averaged 48.4 before surgery and 89.1 at the most recent follow-up examination. Significant improvements in pain, function, and range of motion were observed following total hip arthroplasty. No hip has required a surgical revision or reoperation. Heterotopic ossification was observed in 6 of 14 hips (43%) in the ankylosing spondylitis patients who had not had any perioperative prophylaxis for heterotopic bone formation. By contrast, 43 of 49 hips (88%) demonstrated heterotopic bone formation in a well-defined control group of 45 men with other diagnoses undergoing the same procedure by the same group of surgeons at the same institution. During the same period using the same implants. A thorough review of the literature and data from the institution does not support the notion that ankylosing spondylitis patients are necessarily predisposed to form heterotopic ossification. The use of routine perioperative prophylaxis for heterotopic ossification may not be warranted in all patients with ankylosing spondylitis undergoing routine primary noncemented total hip arthroplasty.

Cementless total hip arthroplasty in patients with ankylosing spondylitis
BACKGROUND The purpose of this study was to document the clinical and radiographic results of a consecutive series of patients with ankylosing spondylitis (AS) who had undergone bilaterally primary THA using non-cemented components. METHODS Twenty-six hips of 13 patients with bilateral ankylosis of the hip caused by AS were converted to THA from January 2000 to January 2008. The mean age of t...
متن کاملTotal Hip Arthroplasty with Modular Stem for Dysplastic Hips in South Asian Population
Introduction: Optimum component positioning and orientation is required to optimize the functional result during total hip arthroplasty for dysplastic hips. Patients and methods: Sixty-two patients (66 hips) including 33 males and 29 females underwent total hip arthroplasty using modular stem prosthesis at an average age of 40.6 years (range 17 to 49 years). Nineteen hips were clas...
متن کامل[Total hip replacement in patients with ankylosing spondylitis.].
The authors present their experience with total hip replacement in patients with ankylosing spondylitis. In the period of 1985-1997 they performed in total 66 total hip replacements in 41 patients. Of the total number of the used implants 24 were fully cemented (the average follow-up - 13 years), 3 were hybrid and 39 cementless (the average follow-up - 6 years). The results of total hip replace...
متن کاملReparative radiological changes of hip joint after TNF inhibitors in ankylosing spondylitis
Background: Hip involvement in ankylosing spondylitis (AS) is a common extraspinal arthritic manifestation, which is associated to a worse functional outcome. Little data are available on the effectiveness of conservative treatment strategies. The TNF inhibitors have been proven effective on AS activity parameters. Their structural effect on hip disease however, little is studied. Case pres...
متن کاملA Single Femoral Component for All Total Hip Replacements Performed by a Trust? Does This Affect Early Clinical and Radiological Outcomes?
Background: Hospitals may be under pressure to implement cost saving strategies regarding prosthesis choice. Thismay involve the use of components which are not the first preference of individual surgeons, or those they have littleexperience with. We aim to examine the effect of standardizing the type of femoral stem used in a single trust, anddetermine whether this is safe practice, particular...
